I wish to apply for basic archery skills training with Melton Mowbray Archery Club. I understand that, when not properly used, archery equipment is dangerous and consequently I must follow the instructions of the instructor/coach/field captain in order to ensure my own and other peoples safety.
If the application is a junior, it is the responsibility of the parent or guardian to see that the junior understands the message above.
If the junior is under 16 years old, it is the responsibility of the parent or guardian to be present during the training.
Please advise the trainer/coach if the applicant has any medical condition or is taking any medication of which the trainer/coach should be made aware of for safety reasons.
Under the provisions of the GNAS Protection of Children and Vulnerable Adults, permission must be sort from the parent or guardian of a child before photographs are taken. Any photos taken at the Club will only be used for publicity purposes and may also be used on the website. Saturday 10th September 9.30 - 12.30
Saturday 17th September 9.30 - 12.30
Saturday 24th September 9.30 - 12.30
